Fixes swamp-club #2852, a follow-up to #2770 (PR #396) from its adversarial review.

After revise → plan → plan-review (open high finding) → rework → plan, the plan stage is handed the plan feedback the last plan already answered, and the prompt told the planner to append it, so feedbackIncorporated (and plan_revised in the Lab) could list the same round twice.

  • The plan prompts in swamp-club-swamp-extensions, build-swamp-extension and starter now carry the last plan's list forward and add the feedback only when it is not already the list's last entry.
  • starters.ts regenerated.
  • factories_test checks each plan prompt for the instruction.

Prompt-only; no engine change. verify-build and verify-reviews passed at 4ebdeab7fbed6a8d18e80d9f5481b6f914861d78, and the attestation is posted.
